SWIM DAY WAIVER

I, the undersigned parent/legal guardian, give permission for my child to participate in Swim Day activities hosted by Fusion Youth Outreach and its affiliates.*

I understand that swimming and water activities involve inherent risks, including but not limited to injury, illness, accidents, drowning, or other unforeseen incidents. By signing this form, I voluntarily assume all risks associated with my child’s participation.*

I agree to release, waive, and hold harmless Fusion Youth Outreach, its affiliates, staff, volunteers, directors, and representatives from any and all liability, claims, demands, damages, or causes of action arising from participation in Swim Day activities*

I understand that:

• Children are expected to follow all safety rules and staff instructions.

• Fusion Youth Outreach and its affiliates are not responsible for lost or damaged personal belongings.

• In the event of an emergency, I authorize staff to obtain medical treatment for my child if I cannot be reached immediately.

In addition to my agreeing to the above waiver for myself; I also fully understand and sign this waiver on behalf of my minor child.

I thereby agree to the same terms and conditions on behalf of my minor child. By checking this box, I constitute that I have voluntarily and knowingly acknowledged and agreed to all terms herein stated.*
