KID'S ZONE WAIVER

Welcome to our Family Fall Festival. Please sign the waiver below if you wish for your child to participate in activities in our Kid's Zone.

I grant permission for photographs and video of my child(ren) to be taken during the event for use in church publications, social media, and promotional materials.

I am and/or giving my child express permission to voluntarily participate in any and all activities at the Community Fair including the KidZone at Victorious Life Family Worship Center strictly at my own risk. I am aware of the risk associated with said participation which may include but is not limited to physical injury, pain or illness.

I understand that any injury or negative outcome that may arise from my participation is my responsibility and mine alone. I therefore assume any and all related risk, both known and unknown to me or my child.

In consideration for participating in the Community Fair Event located at Victorious Life FWC, I hereby, for myself, my heirs, executors, administrators, assigns or personal representatives KNOWINGLY AND VOLUNTARILY enter into this waiver and release liability and hereby waive any and all rights, claims, or cause of action of any kind whatsoever arising out of my or my child’s participation in the Community Fair Event located at Victorious Life FWC, LaPlace, Louisiana.

I do hereby release and forever discharge Victorious Life FWC, Apostle Renita Graber, any affiliates, members, agents, attorneys, staff, volunteers, heirs, representatives, assigns, or successors of any and all liabilities associated with or resulting from any injury or illness or damage I may suffer as a result of my participation in the Community Fair Event including but not limited to the KidZone.

In addition to my agreeing to the above waiver for myself; I also fully understand and sign this waiver on behalf of my minor child.

I thereby agree to the same terms and conditions on behalf of my minor child. By checking this box, I constitute that I have voluntarily and knowingly acknowledged and agreed to all terms herein stated.
