The parent guardian of the child listed above consent to allow HER/HIM to attend and participate in Victorious Life FWC Victory Cup Grand Opening Event:

I EXPRESSLY give my child permission to participate in the Victorious Life FWC Victory Cup Grand Opening Events. This permission includes a release from any liability that may occur while in attendance, while swimming in the pool and/ or participating in other activities that may cause injury or bodily harm.

My child DOES NOT have permission to SWIM but I EXPRESSLY give permission to attend and participate in other activities at the event.

I the parent/guardian will also participate in Victorious Life FWC Victory Cup Grand Opening Event activities. This permission includes a release from any liability that may occur while in attendance that may cause injury or bodily harm.

I the parent/guardian will NOT participate in Victorious Life FWC Victory Cup Grand Opening Event activities.

I expressly consent to the above and expressly hold harmless, free of any and all liability whatsoever Victorious Life FWC, any of its affiliates and/or representatives ( including Pastor, Officers and members).

I acknowledge that I am fully aware of any potential danger or risk that participation in this event may cause to me or my child and I consent of my own free will and accord. My signature signifies that I am the parent or legal guardian with full authority to waive liability and give the above consent.
